What is the "Friends" "Buddy List" thing?

What is the thing about "friends list" and buddy list?? I noticed that folks have a friends list..and several member names are on it. What exactly is this? A secret club? Is there a special handshake?

I always have trouble with that secret handshake ... makes my arthritis flare up.

I have no problem with the handshake, it is the secret dance that kills me..bad knees. LOL
