my friend has a five year old grandson who she says has enough clothes & toys to supply 15 - 5yr old's
i want to buy him a xmas gift & so aware of the above am thinking of something educational, all ideas appreciated

Go to the army surplus store & get him one of those collapsable shovels. It might get him out of the house to play camping or army. It might encourage him to bury all those useless toys!

Give him a box of boxes. one box inside another, inside another, and so on. He's figure out all kinds of ways to use them. And if he doesn't, his mom can use them to pack all those useless toys in to send to the Salvation Army.

Leap frog is a great learing aid.. can be expensive if he doesn't have the book to put the cassettes in to learn.
I suggest "THOMAS THE TRAIN" and a few good book mom or dad can read to him.

How about a children's magazine............ this way he'd get something each month in the mail over the next year
or
We have 'savings bonds' that can be purchased and will mature in the future -- this is the type of gift that he'll really appreciate later, when he's 18 or so
or
invite him out for the day and take him, and only him, someplace special -- eat out on the go (the zoo could be fun) then buy him a tee shirt or something as a momento of the day

Give your friend a gift certificate to a toy store and attach it to a coloring book and crayons, or something like that (This way, the kid has something to open). Let grandmom and grandson go shopping together.
