I think they're piercings originally, and he's gone bigger & bigger like some African tribes do.

The bottom of her lip the ridge even split - she started stretching it with a coin and now thats a plate.

those black disc thing on the original pic locks like hockey pucks.

I watch the Discovery channel on tribes - I find it very interesting - what we see disturbing is art to them and a symbol of status.
