Halfords!

Halfords.com went off the Internet. Website, email, the entire sheebang. Some prat in the IT department failed to renew the domain registration at Network Solutions.
Halfords is not a small business. 10000 employees, over a billion dollars a year in revenue, lord knows how many branches, one of the London Stock Exchange 250. Then bang, emails bouncing and no orders over the web.
If they have an IT Director on the Board I reckon he or she should be fired. If they don't have an IT Director on the Board then the Chairman should be fired for not recognizing the importance of IT.
